Students coming to the Czech Republic to study are obliged to have comprehensive health insurance for the entire duration of their visa/ residence permit. Exception: if a student is officially employed under a labor contract (HPP or DPČ), he/she is automatically included in the public health care system and is not obliged to buy commercial insurance.
A foreigner with a residence permit cannot pay contributions to the public health care system, even if he/she has registered as an entrepreneur (OSVČ). The right to participate in the state insurance only arises if you have a residence permit or EU citizenship. Therefore, such entrepreneurs need to conclude a private comprehensive insurance contract.
Family members (spouses, children, parents) who apply for reunification with a person living in the Czech Republic (foreigner with residence permit or permanent residence permit) must provide proof of health insurance. If they do not work and are not eligible for VZP, they must take out private comprehensive insurance.
As long as temporary protection (dočasná ochrana) is in force, Ukrainians are included in public insurance. However, if a person switches to a special residence permit and does not work anywhere, he or she is obliged to provide a comprehensive private insurance contract.
Foreigners with a work card are included in the public health care system and the employer pays contributions for them. But if a person is dismissed or changes jobs, he or she temporarily loses the insured status. For the period between jobs (usually 1-2 months), the foreigner is obliged to take out private comprehensive insurance for this period.
Children who come to the Czech Republic on a long-term visa are obliged to have comprehensive private health insurance for the entire period. When children transfer to a residence permit, they are included in the public health care system in the same way as Czech citizens.
